The Children and Young People’s Plan refers to all services for children and young people aged 0 – 25 and maternity services.
It is the key document setting out how the well-being of all children and young people in the area will be improved.
The Plan will direct the work of the partners, in line with the United Nations Convention of Rights and the 7 Rights to Action as set out by the Welsh Assembly Government.
Mae’r Cynllun ar gyfer Plant, Phobl Ifainc yn cyfeirio at bob gwasanaeth i blant a phobl ifainc rhwng 0 a 25 oed a gwasanaethau mamolaeth.
Dyma’r brif ddogfen sy’n disgrifio sut bydd modd gwella lles plant a phobl ifainc yr ardal.
Mae'r Cynllun yn rhoi cyfeiriad i waith y partneriaid yn unol â Chonfensiwn y Cenhedloedd Unedig ar Hawliau, a Gweithredu’r Hawliau gan Lywodraeth Cynulliad Cymru.
